Where We Are

US Family Health Plan currently owns and operates three health centers in New York : the Bay Ridge Family Health Center in Brooklyn, The Fort Wadsworth Family Health Center on Staten Island, and the Mitchel Field Family Health Center on Long Island .

Our centers proudly provide primary care services to USFHP members. The offices are staffed by board certified physicians, and knowledgeable and friendly clinical and administrative support staff.
